Application:

Applications are reviewed and selected based on these important factors:

• Applicants ability to take care of a service dog both monetarily and physically (food, grooming, vet care, exercise, continued & follow up training).

• Applicant having realistic expectations of what can and will be trained.

• Applicants need for a hearts healing/life changing dog.

• Dogs come from 4E Kennels where they have been raised on an advanced service dog curriculum and carefully evaluated and selected based on their aptitude to perform the tasks required.

Eligibility:

• APPLICANTS MUST DISCLOSE ALL requested HOUSEHOLD INFORMATION.

• HAVING OTHER DOGS IN YOUR HOME COULD INFLUENCE YOUR ELIGIBILITY.

• APPLICANT/FAMILY MUST NOT HAVE RECEIVED A DONATION FROM ANOTHER LARGE WISH/GIFT ORGANIZATION.

• APPLICANT MUST LIVE WITHIN THE CONTINENTAL UNITED STATES.

• if facility, must provide written permission from location.
